  1. Practice Pelvic Floor Exercises

Strengthening the pelvic floor muscles can significantly improve ejaculatory control. These muscles play a crucial role in ejaculation, and by exercising them regularly, one can enhance their ability to delay orgasm. Kegel exercises are the most common form of pelvic floor exercises and involve contracting and relaxing the pelvic floor muscles repetitively. To perform Kegels, identify the muscles used to stop urine flow midstream and then squeeze and hold them for a few seconds before releasing. Gradually increase the duration and number of repetitions as your muscles become stronger.

  1. Behavioral Techniques

These techniques can be suggested by a healthcare provider or tried independently.

  • Self-Distraction

In moments when arousal levels become too intense and climax seems imminent, take a deep breath and divert your thoughts to something else, preferably something mundane and unexciting. This approach is akin to the well-known pop culture advice of “thinking about baseball.”

Once arousal levels subside while maintaining an erection, you can resume sexual activity.

  • Stop-and-Start Method

If you feel that you are approaching the point of climax, withdraw your penis from your partner momentarily and allow yourself to relax, preventing ejaculation.

Practicing the start-and-stop approach during sexual stimulation can help you prolong the sexual experience and delay ejaculation.

  • Squeeze Method

This method involves either you or your partner gently squeezing the end or tip of the penis with moderate pressure for approximately 10 to 20 seconds when ejaculation is about to occur. After the squeeze, stop all stimulation for roughly 30 seconds before resuming sexual activity.

This process can be repeated as needed until the desired moment of ejaculation. The stop-and-start method can also be combined with the squeeze method for greater effectiveness.

  1. Masturbation

By familiarizing yourself with your own feelings and sensations, you can build confidence in managing your sexual responses.

  • Desensitizing Creams

Additionally, using thicker condoms can also have a desensitizing effect by reducing stimulation, thus prolonging the sexual experience.

  • Oral Medications

For oral medications, selective serotonin reuptake inhibitors (SSRIs) are commonly prescribed on an “as needed” or off-label daily dosing basis to manage premature ejaculation. These medications help regulate serotonin levels, which can contribute to better ejaculatory control.

In cases where premature ejaculation coexists with erectile dysfunction, phosphodiesterase-5 inhibitors like Cenforce (sildenafil) may be used.

  • Combination Approach

Anxiety reduction techniques, coupled with medication, can also be employed to address premature ejaculation effectively.

  1. Focus on Relaxation Techniques

Anxiety and stress can exacerbate premature ejaculation. Incorporating relaxation techniques, such as deep breathing, meditation, or yoga, can be beneficial in reducing overall anxiety and promoting a calm and composed state during sexual encounters. By managing stress, you can enhance your ability to maintain control over ejaculation without erection.
