There’s a reasonable chance you’ve heard of Viagra. It’s been the cultural shorthand for erectile dysfunction treatment since the late nineties, to the point where the brand name basically replaced the condition in everyday conversation. But Viagra isn’t the only option out there, and for a lot of men, it’s not even the best one. Tadalafil, sold under the brand name Cialis, has been quietly building a devoted following for years, and the reasons are fairly practical.
The biggest difference between the two drugs comes down to timing. Sildenafil, the active ingredient in Viagra, works for about four to six hours. Tadalafil stays active in your system for up to 36 hours, which is why it picked up the nickname “the weekend pill.” That’s not marketing fluff. It genuinely does change how men approach the whole thing. You’re not watching the clock, you’re not mentally calculating whether you took it “in time.” For a lot of guys, that shift alone makes a significant difference to how relaxed they feel, and relaxed is kind of the whole point.
How It Actually Works
Tadalafil belongs to a class of drugs called PDE5 inhibitors. Without getting too deep into the biology, what these drugs do is allow increased blood flow to the penis when you’re aroused. They don’t cause an erection on their own, which is a common misunderstanding. Sexual stimulation still needs to happen. The medication just removes a physiological barrier that ED creates.
It typically kicks in within 30 to 60 minutes, though some men find it works faster. Having a heavy meal beforehand doesn’t affect it the way it can with sildenafil, which is another practical plus if you’re planning an evening out and don’t want to be strategically eating a light dinner at 6pm like you’re preparing for surgery.
There’s also a daily dosing option available at a lower dose, usually 2.5mg or 5mg, which some men prefer entirely. It removes the planning aspect altogether. You just take it every day and don’t think about it. This option tends to work well for men who have ED alongside other conditions like benign prostatic hyperplasia, since tadalafil is actually licensed for both.
Who It’s Suitable For
Most men with erectile dysfunction who are otherwise in reasonable health can use tadalafil. The main contraindications are nitrates (commonly prescribed for heart conditions like angina) because the combination can cause a dangerous drop in blood pressure. If you take any heart medication, that’s the conversation to have with your GP before anything else.
It’s also not recommended for men with severe liver or kidney problems, and you shouldn’t take it alongside certain alpha-blockers without medical guidance. These aren’t obscure caveats. They’re the reason why getting this medication from a legitimate source matters, rather than ordering from wherever’s cheapest on a Google search at 11 o’clock at night.
Side effects do exist. Headaches and facial flushing are the most commonly reported ones, and some men notice back pain or muscle aches within a day or two of taking the 20mg dose, which is a slightly unusual side effect profile compared to other PDE5 inhibitors. Most of these are mild and pass fairly quickly. Priapism, a prolonged and painful erection lasting more than four hours, is rare but serious and needs immediate medical attention if it happens.
Getting It in the UK
Tadalafil has been available as a generic in the UK for several years now, which brought the price down considerably. You can get it on the NHS in some cases, or via private prescription, or through regulated online pharmacies and prescribing services. The important word there is regulated. In the UK, any legitimate service prescribing this medication should be registered with the Care Quality Commission and have qualified prescribers doing the actual clinical assessment, not just a questionnaire.
It’s worth treating any site that lets you buy ED medication without any medical review as a red flag. Not because it’s illegal (though sometimes it is), but because the medical history questions aren’t just box-ticking. They’re there to catch the contraindications that could actually hurt you.
ED affects a significant portion of men at some point, particularly from their forties onwards, and the conversation around it has thankfully become far less charged than it used to be. Tadalafil is a well-established, well-studied drug with a solid record.
